In order to proceed with the recognition of higher education diplomas obtained abroad one should submit the following documents:
- completed and signed form,
- diploma of higher education to be recognised*,
- documents enabling the assessment of the course of study, obtained learning outcomes and period of the studies (diploma supplement, transcripts, syllabuses defining learning outcomes, student record book or other documents),*
- certificate, diploma or other document on the basis of which the person applying for recognition of the foreign diploma was admitted to studies completed with this diploma,*
- statement of date and place of birth,
- signed information clause.
*The documents ought to be translated to Polish by a sworn translator included on the governmental list of sworn translators provided by the Ministry of Justice or prepared/authenticated by a competent consul of the Republic of Poland.
A completed form along with copies of the required documents (originals available for inspection) ought to be submitted in the
Didactic Office (building of the Rector’s Office of the Medical University of Silesia)
