Required documents

The candidate who proceeds to electronic registration sets up the account on the SENSUM platform which is available at the address: https://sensum.sum.edu.pl.

After logging in to the Candidate's account, a submission must be formed with all the mandatory fields to be filled out. The next step is to upload electronic copies (scans) of the required documents in the pdf format.

Scans of the documents must be identical to the documents that will be attached to the Application form for diploma recognition.

1. List of required documents:

a) diploma of graduation;

b) documents enabling the assessment of the curriculum of studies, the obtained learning outcomes and professional entitlements, realized practices, duration of studies (supplement to the diploma, list of passed subjects and grades, subject cards determining the learning outcomes, student index card or another document);

c) certificate, diploma or another document on the basis of which the person who applies for recognition has been accepted to studies;

d) statement of the person applying for the recognition concerning the place and time of birth. The statement may be downloaded along with the application form after all its attachments have been approved;

e) documents confirming the change of name and/or surname.

2. Documents translation must be submitted with the application form as per sec. 1, elaborated and certified by a sworn translator or elaborated by a foreign translator and authenticated by the relevant consul of the Republic of Poland.

3. In case if the documents specified in sec. 1 have been issued in a language that is not provided by any sworn translator entered on the list of certified translators maintained by the Minister of Justice of the Republic of Poland, a standard translation into Polish of the documents specified in sec. 1 letter a) to c) must be provided.
